Pregnancy-related pelvic girdle pain (PPGP) is a separate diagnostic classification which may affect over half of all pregnant women with symptoms persisting well beyond the post-partum period. Although largely theoretical, PPGP is currently thought to be due to a diminished capacity for the pelvis to accept and transfer load. Several risk factors have been identified including pre-existing lumbopelvic pain, multiparity, previous pelvic trauma and emotional stress. Further, post-partum depression and delay in returning to has been associated with PPGP. Physical therapists are very well positioned to deliver effective evidence-based care for women with PPGP. This course will present an evidence-based overview of physical therapy for low back/PPGP including pathomechanical background, etiological factors, classification and diagnosis, examination and intervention. Additionally, emerging theories related to pain science, HPA axis dysregulation and sensitization, therapeutic alliance and trauma informed care, will be explored in relation to PPGP. Clinical vignettes will be used as a tool to apply all care concepts and inform physical therapists how evidence-informed cognitive approaches to care “fit” the established science relative to PPGP.

 

Objectives:

  1. Discuss relevant literature on PPGP including etiology, diagnosis, risk factors and prognostic factors.
  2. Justify rational for appropriate examination and intervention strategies (inclusive of primary and secondary prevention).
  3. Integrate pain science informed care strategies with a mechanical approach to PPG including functional outcome measures/questionnaires, examination and interventions.
  4. Apply current evidence-informed care principles for PPGP from a primary health care perspective and with an emphasis on the roles of physical therapists and primary care providers.

 

 

Susan C. Clinton PT, DScPT, OCS, WCS, FAAOMPT, NBHWC is an award-winning Physical Therapist in professional achievement and co-owner of Embody Physiotherapy and Wellness in Sewickley, PA. Susan is board certified in orthopedic and women’s health physical therapy, a fellow of the American Academy of Orthopedic Manual Therapy and a board certified health and wellness coach. She is an international instructor of post-professional education in women’s health (including GI issues in Women), orthopedic manual therapy and business psychology. She is on Faculty at Andrews University in the Doctor of Science in PT program, a Master Coach for the Integrative Women’s Health Institute and a clinical faculty instructor for the North American Institute of Orthopedic Manual Therapy. She is an adjunct faculty at many other universities and is an active professional/clinical mentor.
